Your comprehensive intake assessment — the foundation of your fully personalized program.
This is your comprehensive client intake assessment in which we will ask you to provide highly relevant information which will allow us to get to know you on a deeper level. The answers to these questions are essential in order to allow us to design an optimized and truly holistic program for you. Please answer all questions in the most accurate manner possible while being as concise as possible.
The biomarkers from your labs allow us to gain an understanding of what you are, however, this assessment is our opportunity to gain an understanding of who you are. Please be as open as possible regarding your answers as we will never know what you don't tell us.
The accuracy at which you complete this questionnaire will determine the accuracy of the data we collect. For this reason, it's vital you take your time with each question as this contributes greatly to the quality of your final program design.
Please do not begin this assessment until you have at least 60 minutes of uninterrupted time to do so.
